Touch 3 Adult Programs
Waiver, Release of Liability, and Media Consent
Please read the waiver and release of liability carefully, then submit the form below to confirm your acceptance of the terms. This form is for adult participants. For participants under 18, please use the Youth Release Form.
1. Acknowledgment & Assumption of Risk
I acknowledge that participating in Touch 3 Volleyball LLC Adult Programs (including tournaments, drop-in gym events, open play, leagues, and related activities) involves inherent risks, including but not limited to bodily injury, illness, permanent disability, death, and economic loss. These risks may result from my own actions or negligence, the actions or negligence of others, equipment failure, or facility conditions. I voluntarily and knowingly assume all risks associated with my participation.
2. Health & Safety Obligations
I confirm that I am in good physical health, properly conditioned, and medically fit to compete in high-intensity physical activity. Prior to participating, I will inspect the courts, equipment, and facilities. If I observe any unsafe conditions, hazards, or risks, I will immediately notify event staff or officials and refrain from participating until the unsafe condition is resolved.
3. Release & Hold Harmless
In consideration of my participation in Touch 3 Adult Programs, Touch 3 Tournaments, and Touch 3 Drop-in Gym events organized by Touch 3 Volleyball LLC, I hereby release, waive, discharge, and hold harmless Touch 3 Volleyball LLC, event sponsors, facility owners, venue managers, officers, directors, employees, agents, and volunteers (collectively, the "Released Parties") from any and all liability, claims, demands, or damages for personal injury, illness, death, or property damage resulting from negligence or my participation in any event or activity.
4. Medical Authorization & Emergency Contact
In the event of an injury, illness, or medical emergency during any program or event, I authorize licensed medical personnel, first responders, and event officials to administer emergency first aid, medical treatment, or emergency transportation as deemed necessary. I agree to be financially responsible for all costs associated with such medical treatment or transportation.
5. Media & Image Release
I grant Touch 3 Volleyball LLC the irrevocable, worldwide right to capture, record, edit, and use my image, likeness, voice, or video footage recorded during any event in any media format (including website, social media, print, and promotional materials) for marketing, commercial, or informational purposes without compensation, royalty, or prior approval. I release the Released Parties from any claims or privacy violations related to the use of such media.
6. Rules Compliance & Eligibility
I confirm that I am at least 18 years of age. I agree to abide by all Touch 3 Volleyball LLC program rules, regulations, codes of conduct, and instructions given by referees and officials. I understand that failure to comply with safety rules or respectful conduct guidelines may result in immediate removal and disqualification without refund.
I HAVE CAREFULLY READ AND FULLY UNDERSTAND THIS WAIVER AND RELEASE OF LIABILITY. I ACKNOWLEDGE THAT I AM SIGNING IT VOLUNTARILY AND GIVING UP SUBSTANTIAL LEGAL RIGHTS.
